Lincolnshire Police are appealing for anyone who may have seen a male to contact them.
Trevor, a 55-year-old male, was last seen at Pilgrim Hospital in Boston on Tuesday, 24 September.
He is thought to be wearing a white top with blue horizontal stripes, grey combat trousers and black trainers.
The force are extremely concerned for his welfare and are desperate to track him down.
